Agreement and Initial Steps​

Initial Consultation

Meeting and Assessment: Our onboarding process begins with an initial consultation where we meet with you and your loved one. During this meeting, we conduct a comprehensive assessment to understand your loved one’s needs, preferences, and any specific requirements. This can be done at their current location or at our home, whatever works best and is most convenient!

Agreement

Customized Care Plan: Based on the assessment, we develop a customized care plan tailored to your loved one’s needs. This plan outlines the services and support we will provide.
Signing the Agreement: Once the care plan is agreed upon, we proceed with the formal agreement. This document details the terms of care, costs, and other important information. We ensure that all your questions are answered and that you fully understand the terms before signing.
 
Preparing for Move-In
Pre-Move Coordination

Logistics Planning: Our Care Coordinator will assist you with planning the logistics of the move. This includes selecting a move-in date and coordinating any necessary transportation.
Room Preparation: We prepare your loved one’s room to make it as comfortable and personalized as possible. We offer furnished suites including a motorized adjustable bed, but you are welcome to bring personal items, furniture, and decorations to make the space feel like home.
Medical and Personal Documentation

Gathering Records:

We help you gather and submit all necessary medical records, prescriptions, and personal documentation. This ensures we have all the information needed to provide the best care possible.
Medical Equipment and Supplies: If your loved one requires any special medical equipment or supplies, we coordinate their delivery and setup.
 
Move-In Day
Welcoming Your Loved One

Warm Welcome: On move-in day, our team is ready to welcome your loved one with warmth and care. We understand this can be an emotional time, and we aim to make the transition as smooth as possible.

Orientation: We provide a thorough orientation to help your loved one get acquainted with their new home. This includes meeting staff members, touring the home, and explaining daily routines and activities.
Settling In

Personalized Support: Our team assists your loved one in settling into their suite and making them feel comfortable. We ensure that they have everything they need and address any immediate concerns.
Introduction to Fellow Residents: We introduce your loved one to fellow residents, encouraging social interaction and helping them build new friendships.
 
Ongoing Care and Communication
Monthly Check-Ins

Regular Updates: Our Care Coordinator conducts monthly check-ins to monitor your loved one’s well-being and adjust their care plan as needed. These updates ensure that any changes in health or preferences are promptly addressed.
Family Communication: We believe in maintaining open lines of communication with families. You will receive regular updates on your loved one’s progress, activities, and any significant changes. Our Care Coordinator is always available to answer questions and provide support.
Continuous Improvement

Feedback and Adjustments: We value your feedback and encourage you to share any concerns or suggestions. Our goal is to continuously improve our services and ensure your loved one receives the highest quality of care.
Care Plan Review: Periodically, we review and update the care plan to reflect your loved one’s evolving needs and preferences.
